med is difficulty based. The lower you are the harder it is to med, try medding with lower mana levels and you should get better gains.

btw, is it accurate, that you sometimes fail to enter active meditation (by using skill) at 100.0 ?

Yes.
Especially at 0 mana.

Just like healing, to get the best gains you need to actual have the skill "work" So med goes up better if you drain your mana past a certain level, just like healing GM's fast as heck if you drain someones life to 10 hp or so, then heal them, vs just healing them during sparing.
