Rixus didn't actually steal the account usernames and passwords. He was displeased with Azeroth like wise vice versa. Rixus was the only developer(not a very good one at that) that shard had at that time. Azeroth didn't do anything for that shard besides order Rixus around to develop his vision. Rixus released the source of the IPY scripts and server for anyone willing to download. The username and passwords could be easily retrieved through the saved data allowing one account after the next to be hacked.
When the shard wiped the server still had a huge playerbase. The wipe really didn't deter much players from playing there... Azeroth was probably more corrupt than Rixus with his special interest groups that he had. He even PVP'd with a character that was 8x or 9x GM.
The shard finally declined due to many aspects but the lack of being what it claimed was a huge majority by far. This was quite apparent with EVERY shard that has followed in suit since and even before IPY. Roj, Hubris, Blitz, IPY, ATC, DFI, and DIV are all prime examples of this domino effect.
