If you're really paranoid, you can set up your macro like Pirul suggested in another thread, where you have your "If Health >=X" comes directly before the Targeting of the spell instead of before the casting like most people do out of habit. Then I suppose you could have a variable length of time in between when you hit that health mark and when you actually damage yourself. Regardless though, a lucky EQ between the targeting and the 1 second before damage is going to be able to kill you sometimes.
