Sounds like you would be limited , I almost use every button a keyboard

For tank mage pvp I'm pretty sure it would work. Also, I haven't used one in a long time but I'm pretty sure when you release the button is when the action fires, someone correct me if I'm wrong.

I've actually considered this in the past, but I was thinking 360 controller. Either or can be cabled to the PC, so it should just be the button/control mapping.
The bitch is cursor action for item drag/drop. Perhaps if you assigned left stick to cursor movement and right stick to cursor movement + right click, then you could navigate the screen with left and run with the right.
The limited number of buttons simply means alt buttons would have to be used. For example, holding right bumper, or pressing it to alternate to a different 'set' of buttons to be used. Consider the bumpers like Alt/Ctrl and you've got a good amount of room to work with.
A lot of things can be alleviated with razor, so once you have the profile set up for it with appropriate macros, it's just key combinations to get familar with.
For a tank mage, push the left stick to toggle hally, A for Harm, B for EBolt, Y for Explo, X for Recall macro ;P
Just an example, but I think it could certainly work well for a dexxer. Weapon toggle button, heal button, potions and targetting nearest/attack lasts.
It's certainly do-able but we'd really need a custom bit of software to make it work and I don't know shit about programming
