Poison scaling? what increments?

Anyone know how the poisoning chance to hit on weapons incriments?

I know the wiki skill says it is (poisoning skill / 4) but I'm trying to figure out how this actually incriments in the game?

For example:

95 poisoning = ?? % chance to hit ... 23.75%? 23%? 20%?
