#1. Increasing or decreasing INT should (and did) have an effect on mana regeneration speed.

"Your mana regeneration rate is now dependent on your level of Meditation skill, your intelligence stat, and the amount of armor you are wearing. This is termed the "passive use" of the skill. "

#2. Increasing DEX to 120 should have an effect on swing speed.

It does sometimes, depending on which weapon. Everything time wise is calculated by rounding to the nearest tick, which is undisputable.

Secondly, no swing can be faster than 5 ticks(1.25s).

Aye, these are both in effect, Int is responsible for mana rate up to a max rate of 1/sec. Stamina determines swing speed up to a max of one swing per 1.25 seconds.
