Disable music.

The effect you speak of is a direct result of Dagon's music patch.
The original client used MIDI causing other issues on Windows 7 PC's.

Another option would be to use a later client which come natively with digital music and does not suffer the issue you speak off.

By default the limit is 3 however for households the limit can be raised to 4. You will need to contact Derrick for that.

I'm not sure on this one, but the client constantly updates the volume itself. Try modifying the volume level using the uo client options.

In the event your graphics card can't handle this game, try lowering your resolution from 32-bit colours to 16-bit colours.

Where are you downloading from? Did you try downloading from a different location? As far as my experiences goes with that kind of error, it really means the image got corrupted. Are you certain your PC is virus free?

Edit: that sort of error can also occur if you have bad RAM
